The Locals focus is on **care and support** for people aged 26 years and over who are experiencing mental health concerns - including people with co-occurring alcohol and drug treatment and care needs and their family, carers, and supporters.

We are seeking an **experienced, skilled **and **passionate Social Worker**, to join the **Towards Change** **team**. You will be a crucial part of the Victorian mental health reform work which places the community in the centre of the Local Mental Health and Wellbeing network across Victoria.

You will possess a comprehensive understanding of mental health disorders, assessment techniques, and evidence-based interventions, with in-depth knowledge of trauma-informed practice and proven experience utilising trauma-informed, goal and recovery-focused, person-centred approaches in clinical practice.

You will have a background working with diverse populations of individuals experiencing mental health challenges within a **community health** service setting.

The role is **full time, ongoing** and based mainly at our Lilydale sites plus other sites as required (Yarra Junction and Belgrave).

**About the role**:
The Social Worker will work alongside the individual consumers and their families and carers with an individual plan designed to promote, foster and sustain personal recovery.

You will play a vital role in ensuring that participants, carers, and their families receive valuable and supportive mental health care. You will be part of a multidisciplinary team of mental health and wellbeing clinicians to provide a “new front door” service to all adults seeking mental health interventions within the Yarra Ranges Government area. You will play an integral role in changing and shaping the way mental health and wellbeing services and supports are delivered to the community.

**Position Responsibilities**:
**In the role you will**:

- Develop individualised wellbeing plans with participants, families, and teams to address mental health concerns and promote recovery goals.
- Facilitate psychoeducational, support and therapeutic groups/workshops.
- Provide education on mental health, symptom management, medication adherence, and relapse prevention.
- Empower participants to self-advocate and navigate complex systems.
- Conduct comprehensive psychosocial assessments to identify strengths, needs, and risks.
- Demonstrate relevant family violence practice according to MARAM, FVIS/CISS, and local guidelines.
- Provide support and mentoring to junior social workers and students.
- Coordinate/participate in case conferencing, care coordination, and reviews.
- Participate in individual and group supervision, team meetings, clinical reviews, actively participate in skill building activities
- Provide services and support at different locations across the Yarra Ranges, dependant on community need.

**About You**:

- Comprehensive understanding of mental health disorders, assessment techniques, and evidence-based interventions.
- Knowledge of relevant legislation, ethics, and service delivery models.
- Exceptional communication, (high proficiency in written and verbal), interpersonal, and advocacy skills for collaboration and participant engagement.
- Demonstrated ability to conduct comprehensive psychosocial assessments and develop individualised care plans.
- Commitment to high quality care and ongoing professional development.
- Work with individuals holistically and contextually, considering systems, institutions, families, cultures, communities, and ethnicity.
- Strong ability to convey warmth, openness, and concern for welfare of participants.
- Ability to self-regulate, self-monitor own behaviours and practice self-reflection to understand your own personal strengths, limitations and biases.
- Work as part of a multidisciplinary team to contribute to service and team development and the promotion of the Local and its services.
- Capability to make informed decisions on reasonably complex issues, aligning with established policies and procedures, and adept at identifying and resolving problems or obstacles in collaboration with team members.
- Capacity to work autonomously and collaboratively within a multidisciplinary team, efficiently managing multiple tasks in a demanding environment.
- Ability to work respectfully and creatively with a CALD client population, Indigenous Peoples and the LGBTIQA+
