Applications are invited for an exciting permanent grade 2 role in the Rapid Access Discharge Unit (RADU) at Western Health (Sunshine Hospital) which aims to facilitate best care through rapid assessment and discharge for patients admitted to this service. Working collaboratively with the multidisciplinary RADU team, this role will be supported by a grade 3 RADU Allied Health team leader and a comprehensive allied health supervision structure. The RADU unit will offer a 7 day a week allied health service with extended hours of care.

Key components of the role include: a modern innovative RADU model of care clinical caseload, an opportunity to be involved in quality improvement and service development, and the supervision and development of allied health students.

To be successful in this role you will:

- Demonstrate commitment to provision of Social Work practice
- Be able to demonstrate knowledge and experience in acute service provision and discharge planning
- Have strong collaboration skills
- Have demonstrated experience working in multidisciplinary teams
- Demonstrate an understanding of the RADU model of care
- Have experience in quality improvement and/or research projects
- Flexibility to work a rotating 7 day rotating roster, extended hours, weekends and public holidays as required
