Triage Consultant

**About the role**
A rewarding opportunity for a passionate Dementia professional to join the Dementia Support Australia (DSA) services team at HammondCare on a** Fixed Term Full-time Contract until June 2025 **with flexibility to work across a variety of shifts throughout the week at our Malvern site.

**Responsibilities**
- Provide dementia-specific behaviour management recommendations over the phone, via live chat and through phone support case management.
- Provide advice to reduce the impact of behaviours on people living with dementia, their carers and staff.
- Work closely with colleagues and teams to provide information and data which reports statistics and information collected regarding calls and live chat.
- Implement service improvement solutions across the phone and live chat team consistently and efficiently.
- Flexibility to work across a variety of shifts throughout the week.

**What do we need from you?**
- Expertise in supporting people living with dementia
- Experience in Australian residential aged care and or community aged care
- High level of negotiation, communication and relationship building skills
- Excellent problem solving, analytical and decision-making skills
- Ability to prioritise multiple tasks and meet deadlines
- Excellent written and verbal communication skills
- High attention to detail and accuracy
- Empathy and patience
- Relevant qualifications in Allied Health, Nursing, Aged Care or Mental Health highly desirable
