**Description**:
- Build connections with students and safe relational culture at school;
- Student wellbeing check-ins at school, via phone or correspondence;
- Provide students with information, advocacy and referrals to external supports;
- Build confidence, capacity and skills in students to develop self-advocacy;
- Collaborate and liaise with students’ external support providers to maintain a network of support for student wellbeing (e.g., caseworkers, DCJ, SHS);
- Review educational support plans with students and staff (Individual Plans, Adjusted Timetables, Attendance Plans, Behavioural Support Plans, Exemptions);
- Follow up with students who are at risk of disengaging.
- Provide interim support and refer students experiencing crisis to relevant crisis services;
- Follow relevant school procedures to ensure students and families have support as needed during crisis. Work with student, Head of Campus, Principal, Head of Narnia and Narnia Wellbeing Officer and external support services as appropriate to create Safety Plan and collate necessary documentation (AVO, persons of concern, legal documentation);
- Weekly collaborative meetings and correspondence with Narnia Director and Narnia Wellbeing Officer regarding the support, safety and wellbeing of families;
- Plan collaboratively with Narnia regarding joint wellbeing activities
- Facilitate provision of external services where possible at school;
- Attend relevant Interagency meetings;
- Educate students on local community support, opportunities and organisations;
- Attend initial Interview or Initial Individual Plan meeting and liaise with Narnia and external supports;
- Ensure support plans are in place for students' first days at school. Communicate relevant information and needs of students with Young Parents Staff;
- Support our young parents to help break unhelpful cycles of poverty and unhealthy relationships;
- Help build positive family, relationship and community connections;
- Help develop mental health and spiritual growth;
- Assist students to build skills, knowledge and confidence to make good decisions and advocate for themselves and their children;
- Demonstrate an ongoing commitment to personal and professional development through attending courses, supervision and group supervision;
- Prepare for regular professional supervision sessions and peer/group supervision;
- Participate in essential or mandatory training and remain current with child protection knowledge;
- Be aware of and adhere to all policy and protocol requirements.
