**The role**:
The NDIS Support Coordinator is a front-line position that is responsible for delivering exceptional customer service and supporting NDIS participants to allow them to understand and utilise their funding in order to achieve their goals. The Support Coordinator may work independently of co-workers or supervisors and therefore must possess good judgement and take initiative whilst following policies and procedures of the company. You will support participants to understand their plan, funding and supports available to them, Arrange for any assessments required to determine the nature and type of funding required. Coach and support clients to build their confidence and skills to implement their plan and receive the most out their NDIS plans and assist clients in preparing for plan review meetings by considering new goals, identifying solutions to any problems experienced within the plan/budget and assess whether they achieved their goals with the plan/funding available to them.

Your key responsibilities will include:

- Conducting initial evaluations: Engage with participants to identify their requirements and aspirations.
- Developing personalized support plans: Collaborate with participants and their support network to create individualized plans.
- Identifying and coordinating service providers: Determine suitable service providers and facilitate the implementation of service agreements.
- Providing ongoing support and guidance: Offer continuous assistance and advice to participants and their support network to ensure goal achievement and adherence to the support plan.
- Monitoring participant progress: Track and evaluate participant progress, making necessary adjustments to the support plan when needed.
- Maintaining accurate records: Keep detailed and up-to-date records of participant progress and the delivery of services.
- Effective communication: Establish clear and effective communication channels with participants, their families, service providers, and other stakeholders to ensure everyone is informed and involved in the support plan.
- Professional development: Engage in continuous professional development to stay updated on NDIS policies and procedures.

**Essential Criteria**:

- Minimum 2 years experience as NDIS Support Co-Ordinator.
- Demonstrated knowledge and understanding of the NDIS and its policies and procedures.
- Strong communication skills, customer service skills, and demonstrated problem solving skills.
- Ability to manage a diverse case load.
- Ability to work independently and as part of a team.
- Current Drivers' License.
- Access to fully insured car.
- NDIS Worker Check and Orientation Module or willingness to complete.
