**About the role**:

- Act as first point of contact for people to speak to about mental health concerns, receive information and referral to appropriate services
- Provide holistic assessment of needs using Commonwealth Initial Assessment Referral Decision support tool
- Ensure a consumer centred approach is used for all mental health referral enquiries
- Proactively work towards achieving individual and team goals while promoting a diverse and inclusive environment for all staff and individuals
- Support the continuity of care and information flow between consumers, internal teams, service providers, primary care and other agencies.
- Complete other reasonable duties as required.

**About You**:

- Must be a social worker, psychologist or similar, or an RN with a mental health qualification
- Significant experience in a human services environment, particularly understanding service delivery
- Extensive practical experience in working with people in mental health or related context
- Experience in problem solving and decision making in order to prioritise and coordinate support for client with complex needs
- Professional phone manner and exemplary customer service skills
- Able to work within a team environment and with complex clients
