**Manager - Grampians Aged Care Assessment Service**
**Full-Time (40hrs per week and monthly ADO)**

**About the role**

The Grampians ACAS Manager is a senior health professional with a relevant clinical discipline background who demonstrates excellent interdisciplinary leadership skills. The Manager will guide the professional performance of the interdisciplinary team and ensure key performance indicators and targets are achieved. The Manager is responsible for all aspects of service delivery, including financial management, staff supervision and support, quality improvement activities, as well as displaying skills in innovation, adaptability and flexibility.

**Key Responsibilities**
- ACAS and RAS services meet all the criteria required by Grampians Health (GH) governance & quality frameworks including the National Aged Care Assessment Program Guidelines.
- Implementation of the Aged Care Reforms 2024 and transition to the Single Assessment Tool/‘Support at Home’ In-Home Aged Care model
- Provides expert clinical knowledge and direction to ensure that clinical standards, policies, and procedures promote a consumer centred model of care.
- Monitor assessment waiting lists and implement strategies as required to ensure timely access and oversee delegation processes within the service.
- Grampians ACAS representation at the Victorian ACAS Manager meetings and forums

**Skills and experience**
- A tertiary level qualification in a core ACAS discipline (nursing or allied health) and current AHPRA Registration
- Minimum 3yrs experience in a clinical leadership/management or supervisory role within a multidisciplinary team
- Post Graduate qualifications or working towards in management/leadership (Highly Desired)
- Demonstrated senior level experience and advanced clinical knowledge in:

- Assessment, Care Planning and Discharge planning for older people
- Complex patient care, clinical reasoning, and management skills
- In-Home/Community and or Residential Aged Care
- Experience and skills in leading teams to deliver high quality clinical care to complex consumers ensuring patient safety.
- Demonstrated financial management experience.
- Demonstrated commitment to continuing professional development.
