**Hourly Rate**:$51.76 per hour

**The Position**

As a Chronic Disease Registered Nurse Care Coordinator, you will play a pivotal role in delivering tailored care, helping patients manage their health from the comfort of their homes. You’ll provide expert support in navigating their care, bridging gaps, and promoting self-management strategies that lead to positive health outcomes.

**What You’ll be Doing**
- Conduct comprehensive health assessments, providing tailored recommendations for chronic disease management.
- Deliver care coordination services to at-risk patients, ensuring smooth transitions between care settings.
- Foster collaborative relationships between general practice, community health services, and acute care providers.
- Improve patient experiences, quality of life, and reduce preventable hospitalisations through targeted interventions.
- Champion inclusive care for diverse communities, including LGBTIQA+, CALD, and Aboriginal and Torres Strait Islander people.
- Engage with patients to set health goals, develop care plans, and support self-advocacy.

**What You’ll Bring**
- ** Qualifications**: Bachelor of Applied Science (Nursing), Division 1 Registered Nurse with current AHPRA registration.
- ** Experience**: Advanced clinical skills in managing chronic diseases and complex care coordination.
- ** Diversity Commitment**: Experience working with diverse communities and a commitment to inclusive healthcare practices.
- ** Communication**: Excellent interpersonal skills with the ability to collaborate across a wide range of clinical settings.
