**Case Manager (Clinical Background)**

Are you a passionate and experienced Registered Nurse seeking a rewarding role where you can truly impact the lives of others? We are looking for a dedicated Case Manager with a strong clinical background to join our dynamic team.

About the Role:
As a Case Manager, you will play a pivotal role in providing comprehensive care coordination and support to our consumers within the HCP and CHSP programs. Your expertise in Care Plan assessment, coupled with your clinical knowledge, will be instrumental in ensuring that our consumers receive the highest quality care tailored to their individual needs.

Key Responsibilities:

- Conduct comprehensive Care Plan assessments to accurately identify consumer needs and develop personalized care plans.
- Coordinate and liaise with healthcare providers, family members, and community resources to ensure seamless care delivery.
- Monitor and evaluate consumers’ progress, making necessary adjustments to care plans as needed (example Clinical Procedure as wound care).
- Provide education and support to consumers and their families regarding self-management and disease prevention.
- Advocate for consumers' rights and interests, ensuring they receive the appropriate services and support.
- Train and supervise support workers to ensure they understand and implement care plans effectively.
- Roster support workers shift to optimize care delivery and minimize disruptions to consumers.

About You:

- Registered Nurse
- Must be an Australian citizen or hold a valid working visa
- Minimum of 3 years of clinical experience in a relevant healthcare setting
- Proven experience in Care Plan assessment and case management
- Strong interpersonal and communication skills, with the ability to build rapport with consumers, families, team members and healthcare providers
- Excellent organizational and time management skills.
- Passionate about providing high-quality care and making a positive difference in the lives of others.
- Possess an enthusiastic "CAN DO" attitude.
- Arabic Speaker

**Job Types**: Full-time, Permanent

Pay: From $80,000.00 per year
