**Position Purpose**:The Team Leader is responsible for providing direct supervision and regional

support to Case Managers with the knowledge and expertise of evidence
- based practice, to deliver high quality and consumer directed care for clients
receiving Home Care Package (HCP) services in the community.

This role also support financial reconciliation, business and marketing
strategies as well as promoting innovation in care to deliver the best services
and optimise operational efficiency.

- Full COVID vaccination
- Drivers licence

**Desirable**:

- Relevant Post Graduate qualifications in community, management or aged
care or working towards.

**Essential**:

- Experience in Team Leadership within Community, Healthcare or Aged Care sector- Experience and knowledge within the Aged Care sector, including Home Care Package (HCP) and

Fee for Service Programs (FFS)

**Position Description**

- Demonstrated current specialist knowledge and skills in the provision of evidence-based services
- to clients with complex health needs in a community setting- Highly developed communication skills including negotiation, conflict resolution, supporting
- change processes and creative problem solving- Experience in Client Management Systems (CMS) utilising data and reporting on service outputs
- and outcomes- Broad understanding of a range of both therapeutic and capacity building approaches applicable
- to the populations seeking support under provisions of HCP program- Demonstrated capacity to support business development and growth

**Desirable**:

- Fluent in a second language other than English**Responsibilities/Duties**
Quality Service - Support program development and growth by educating and informing

potential clients and relevant stakeholders of Mayflower’s Home Care
Services
- Provide regular clinical and operational supervision to Case Managers

encouraging exceptional service delivery and adapting work practices to
address significant changes in the sector
- Manage rostering with direct reports
- Review and develop processes relevant to case management that ensure

HCP and program services, legislation, clinical standards, client safety and
program target are achieved

management to deliver seamless consumer directed care and reablement
opportunities
- Identify and mitigate risk with early prevention strategies
- Ensure the case management team recognise, respond, and escalate
- changes to client’ conditions and manage them accordingly- Exercise judgement within the framework of operational and budgetary

guidelines, relevant standards, and legislation when providing case
management services
- Develop and maintain positive working relationships with key partners and

stakeholders such as subcontracted providers, referrers, and discharge
planners
Reporting and - Actively perform clinical documentation as per ISBAR Guidelines.
Documentation - Participate in the auditing and monitoring of functions within the
- department ensuring quality and accreditation standards are achieved- Respond to queries or incidents within the department in a timely manner
- and reporting to SIRS- Provide written report outlining narrative and statistical data
- Report any maintenance issues pertaining to Home Care Services and
