**Dementia Support Worker**

**About us**:
Wallsend Aged Care Facility is located in Wallsend, a lively suburb of Newcastle close to a variety of shops and restaurants. The 98-bed facility is supported by a modern multidisciplinary team of healthcare professionals.

**About the job**:

- The Dementia Advisory Service is a team of two (2) staff located on the Wallsend Campus.
- DAS is part of the CACS GNS Aged Care Services. Other teams within the Aged Care Services include Aged Care Assessment Team (ACAT), Regional Assessment Service (RAS) Community Dementia Nurses, Neuropsychology Service and Hunter Carer Support Officer.
- In this role you will support and provide information to carers of people living with dementia and people with dementia. Provide dementia education to various stakeholders.
- This position helps promote local community awareness of dementia, available services etc.
- You will lead local dementia carer support groups.
- Upon commencement of this role you will be provided with orientation and will receive ongoing support from CNC Dementia and CNC Aged Care.

**Requirements**:

- Registered Nurse, Occupational Therapist, Social Worker, or Physiotherapist with appropriate qualifications and/or AHPRA registration and/or membership to relevant professional body in NSW.
- Eligibility to drive in NSW.
- Experience working with people living with dementia and their carers.
- Ability to prioritise work load within a small team.
- Demonstrate excellent written and verbal communication skills e.g. presentation skills, working with diverse groups.
- Demonstrate professionalism in an empathic and caring manner.
- Understanding of dementia and ability to provide support for people with dementia, their families and carers.
- Understanding and provision of person-centred care in collaboration with clients, carers and service providers to achieve identified outcomes.

**This is a Category A position**. Acceptable proof of COVID-19 vaccination is the Australian Immunisation Register (AIR) Immunisation History Statement (IHS), or AIR COVID-19 Digital Certificate, or AIR COVID-19 and Influenza IHS. Booster doses are highly recommended for all health care workers who have completed the primary course of COVID-19 vaccinations.
