- **Salary**:RN/M2C $82,398 to $104,154 p.a. (pro-rata)**:

- **Permanent Full time position (part time hours to be considered), working over a 7 day roster**

**Overview**
- The Pathways Coordinator applies clinical experience, knowledge and professional competence to contribute to seamlessness of the consumer journey. Pathways Coordination through advocacy, liaison, coordination and allocation of beds includes movement between facility and home based services in a stepped model of care liaising with all key stakeholders relevant to the mental health consumer’s journey.

The Pathways Coordinator as an experienced and competent mental health clinician ensures appropriate access and entry to services for people with mental illness living within rural and remote LHN boundaries are based on clinical need. High level communication, liaison and relevant information sharing with a range of stakeholders across the interfaces of a consumer journey provides a specialist service to consumers, their carers and families.
