**Your role**

Australian Unity is excited and proud to be partnering with Ramsay Connect to deliver the **Diabetes Using Our Strengths Service (DUOSS) **pilot research program. The DUOSS is an innovative virtual and face to face service that incorporates remote patient monitoring, optimising management of diabetes medicines, education, and group yarn ups to help Aboriginal and Torres Strait Islander Elders to manage their type 2 diabetes.

The **Credentialled Diabetes Educator **will lead the delivery of culturally safe and appropriate diabetes care for First Nations peoples enrolled in the Diabetes Using Our Strengths Service (DUOSS) in Bundjalung County (Tweed Heads). The role will work within a supportive, multidisciplinary team to provide culturally appropriate clinical care to improve patient health outcomes. Using their strong interpersonal and collaborative skills they will help improve the health outcomes of Elders enrolled in the service working in collaboration and partnership with the DUOS team, local healthcare providers and organisations.

This is a **part time (up to 0.7FTE) fixed term role of 18 months**, with some travel to Bundjalung Country (Tweed Heads) required.

This project is funded by MRFF funding from the Australian Government’s Targeted Translation Research Accelerator program, delivered by MTPConnect.

**About you**:
As the Credentialed Diabetes Educator, you will be delivering majority of care virtually and will:

- Titrate and/or optimise diabetes medicines with authority from the Elders’ GP
- Provide tailored education to Elders based on their clinical and cultural needs, informed by Remote Patient Monitoring
- Care Coordination
- Management of patient monitoring data
- Provide advice and input for clinical escalations as a result of remote patient monitoring results and/or questionnaire results

**Your expertise and qualifications will include**:

- AHPRA registered with relevant qualification as a credentialed Diabetes Educator with currency of practice
- Knowledge of the health issues affecting Aboriginal and Torres Strait Islander people with strong commitment to improving health outcomes
- Strong interpersonal skills with experience working in a multidisciplinary team to deliver outcomes for complex patients

**Desirable**:

- Previous experience working with Aboriginal and Torres Strait Islander people and/or communities
- Experience in working in virtual care
- Living on Bundjalung Country (Tweed Heads)
