**Overview**

We are looking for a Case Manager, Home Care Package (HCP) to ensure new and existing consumers receive quality service suitable to their needs.

The Case Manager, Home Care Packages provides high-quality home-based care to Home Care Package (HCP) Program consumers, based on the philosophy of supporting individuals to remain living at home, and to enable consumers to have choice and flexibility in the way their aged care and support is provided at home.

The Case Manager will work with consumers, their family, advocate, or carer to identify goals that will form the basis of the Home Care Agreement and Care plan. They will also, under guidance, utilise their professional judgement to design flexible and appropriate solutions and tailor service delivery to meet the consumer’s unique needs within the boundaries of budget allocation and agreed risk parameters.

**Eligibility**

The successful applicant will have the following skills:

- Good Communication
- High level interpersonal and holistic assessment
- Flexibility, prioritising tasks and good time management

A Certificate 3 qualification in Health, Aged Care or Disability or Certificate level 3 equivalent or above is desirable.
