**Support Workers Needed**

We are seeking reliable, friendly, experienced individuals who are looking for, but not limited, to sleepover shifts as a NDIS Support Worker.

At Change Care we believe in the importance of creating a caring community that all can enjoy. We focus primarily on skill building for participants going through life transitions as well as providing daily assistance for participants at home or within their local communities.

**About the Role**
- This role of the support worker is to provide exceptional service whilst supporting Change Care’s clients to meet their goals as set out in their NDIS plan. This includes:_
- Working autonomously to provide appropriate levels of support to maintain and improve the client’s level of independence. This may include community participation, skill building, vocational preparation, relationship building, and various other needs.
- Respond to the client’s needs and collaborate positively with their families, support people and others in their lives.
- Keep accurate and complete records of activities via progress notes
- Work within the NDIS Code of Conduct

**Who you are**
- To be successful in this role we are looking for someone who is reliable and consistent, and the following attributes:_
- Working with Children Blue card
- NDISWC Yellow Card
- Completion of NDIS Worker Orientation Module
- First aid + CPR Certificate
- An understanding of NDIS and a commitment to its principles
- Ability to work in partnership with NDIS participants to achieve their goals
- A passion for assisting individuals with a disability to improve their quality of life and to reach their potential
- Driver's Licence and own car, and a willingness to use for transporting clients (where required)
- COVID-19 Vaccination, it is a Queensland Government requirement that all individuals working within Healthcare Settings, including Disability Support Services are vaccinated against COVID-19, or evidence of medical exemption.

**Job Type**: Casual

**Salary**: From $28.95 per hour

Schedule:

- Afternoon shift
- Evening shift
- Every weekend
- Morning shift
- Night shift
- Public holidays
- Weekend availability

COVID-19 considerations:
COVID-19 Vaccination, it is a Queensland Government requirement that all individuals working within Healthcare Settings, including Disability Support Services are vaccinated against COVID-19, or evidence of medical exemption.
