**The organisation we are representing is looking for an experienced Case Manager for Home Care Packages, based in Belmont.**

The role:
The Case Manager will act as the key contact person and advocate for clients in facilitating the identification of their goals and development of their care plans. Working within a multidisciplinary team, to develop the goal directed plan in conjunction with the client and to respond to client's individual needs and preferences within budget. The provision of creative options and solutions that deliver exceptional client outcomes to ensure that the delivery of services meet all associated funding agreements, professional and legal standards.

To assist the Rosterers and Program Manager in providing, mentoring and supervision to Home Care Workers and promote a harmonious team approach to the delivery of community services. Promote and participate in quality activities, and provide outstanding customer service maintaining friendly, supportive and professional relationships with clients, their relatives and friends. Support colleagues by sharing knowledge and expertise.

Key duties:
**You will have**:

- In-depth knowledge of the Aged Care Sector
- 6 months-5 years of experience managing Home Care Packages or Service Coordination
- Demonstrated knowledge of Referrals and care planning
- Work in the office 3 days a week.
- You will also be willing to undergo NDIS Screen, police, and Working with Children Checks, as well as provide 2 recent managerial references._
