**The Opportunity**

The Newstead Residences is a luxury retirement living community in the heart of Newstead, Brisbane. We have an opportunity for a senior Registered Nurse to join our high-performing team as a **Case Manager**. In this role, you will partner closely with the Care Manager, residents and external providers to deliver and manage personalised care plans for each resident, in accordance with their home care package or fee-for-service requirements. You will enjoy working closely with our excellent care delivery team, liaising with residents, their families and external health professionals on a regular basis, to help enable a retirement lifestyle full of choice, security and enjoyment. This is a **permanent full-time position working Monday-Friday** 8-4pm (hours may be adjusted for the right person)

**You will**:

- Establish comprehensive home care plans aligned to resident care needs
- Complete regular scheduled reviews of all current residents and new residents
- Deliver excellent customer service to residents, families, staff, management, visitors and external health professionals (e.g Allied Health providers and GPs)
- Manage the budget of residents' Home Care package and ensure funding is being used appropriately for the needs of the residents
- Assist the Care Manager in managing the work practises of the care team (e.g PCAs), providing direction as required and assisting with operational tasks such as HR issues & hiring.
- Ensure all residents' preferences and lifestyle wishes are respected and promoted

**What you will bring**:

- Current AHPRA Div 1 registration with minimum two (2) years in Nursing experience, preferably in Home or Community Care
- Demonstrated experience in understanding and managing care plans and clinical assessments
- Solid understanding of Home Care Packages, funding models and fee services
- Excellent people management, technology and communication skills
- Self-motivated individual with a practical and professional approach to management
- Current Police Check, CPR and COVID Vaccinations up to date (required)
