Jasmine Care Pty Ltd is a leading disability support service provider, dedicated to enhancing the lives of individuals with disabilities. We are currently seeking passionate and dedicated Support Workers to join our team in **South Brisbane / Gold Coast Team**. As a Support Worker, you will play a crucial role in providing quality care and support to our clients, promoting their independence and overall well-being.

**Responsibilities**:

- Provide one-on-one support to clients with disabilities in their homes or within the community.
- Assist clients with daily living activities, including personal care, meal preparation, medication management, and household tasks.
- Implement individualised care plans and support strategies, ensuring the physical, emotional, and social needs of clients are met.
- Promote independence and community participation by encouraging clients to engage in social, recreational, and educational activities.
- Maintain accurate documentation of client progress, activities, and any incidents or concerns.
- Collaborate with a multidisciplinary team to ensure a holistic and person-centered approach to care.

**Mandatory Requirements**:

- Valid QLD Drivers Licence.
- Access to a reliable vehicle with fully comprehensive insurance and registration.
- Current First Aid and CPR certificates.
- NDIS Worker Screening Card.
- Current National Police Check.

**Desired Qualifications and Skills**:

- Experience working in the disability support sector or a related field.
- Understanding of person-centered care principles.
- Excellent communication and interpersonal skills.
- Empathy, patience, and a genuine passion for supporting individuals with disabilities.
- Ability to work independently and as part of a team.
- Flexibility in working hours, including the ability to work evenings, weekends, and public holidays if required.
