**A unique opportunity available for a credentialed Diabetes Educator to work within a community setting in QLD or NT.**
**JOB DETAILS**:
A clinical role, providing high-quality and individualised diabetes management/care for patients and their families suffering from, or at risk of, diabetes and associated conditions.
This is a fantastic opportunity to use your skills and experience in diabetes management to provide much needed education and clinical support in a remote Aboriginal community settings (QLD or NT opportunities available)
- Working collaboratively with all health centre and specialist staff to ensure appropriate planning and implementation of care for patients receiving care for their diabetes and associated conditions;
- Providing clinical leadership to other health professionals, including Remote Area Nurses (RANs), Aboriginal Health Practitioners, and medical and allied health professional staff in the area of diabetes management;
- Managing relevant Medical Benefits Scheme claims for care provided as required and applicable; and
- Promoting a multidisciplinary team approach to decision-making and care provided.

**REQUIREMENTS**:
- Must hold an accredited Diabetes Educator certification (AHPRA registered)
- Demonstrated professional experience in diabetes and chronic disease management
- Ability to obtain a National Police Clearance / Working with Children's check
- Must have experience in a clinical setting & community is desirable
- Demonstrated ability to use effective communication skills
- Ability to build rapport quickly & maintain relationships with all clients
