Position Responsibilities
- Support clients to participate in age appropriate activities and tasks including in _home_ support and community access.
- Support clients to access activities and outings in the community.
- Provide direct assistance to each _client_ in accordance with their service plan and as directed
- _ Work_ in accordance with the medication administration policy, _work_ health and safety procedures and _client_ right’s policies including privacy and confidentiality.
- Monitor the safety and well-being of the clients and report any concerns to the Manager.
- Ensure completion of appropriate documentation. Relevant information on support provided is to be recorded in _client_ notes and communication book as required.
- _ Work_ with clients, family/ carers, relevant case managers and other professionals to support the client’s individual goals.
- In conjunction with other members of staff assist in developing, implementing and evaluating training and behaviour programs to attain identified individual goals.
- Ensure that all programming undertaken is appropriately maintained, documented and reviewed.
- Assume the role of key worker for designated clients, including the preparation of a monthly report on client progress.
- Liaise extensively with other staff to ensure the overall smooth and consistent running of the service.
- Participate in Performance Development meetings to identify ongoing training and support opportunities.
- Notify the service if you are unable to _work_ your rostered shifts, with as much notice as possible.
- Take reasonable care for his or her own health and safety
- Take reasonable care that his/her acts or omissions do not adversely affect the health and safety of other persons.
To be considered in this role:
- Certificate IV in Disability, Community Service, Individual Support, Age Care or willing to _work_ towards the same. We can help you through the process
- NDIS Worker Screening Check
- National Police Check
- Working with Children Check
- First Aid certificate and CPR
- NDIS Workers Orientation
- Eligibility to _work_ in Australia
- Fully COVID Vaccinated with evidence
- Current full Driver’s license
Competencies and Experience:
- Previous experience in a similar role - Minimum of 3 months relevant experience
- Previous experience supporting People with Disability.
- Demonstrated proficient written, verbal and interpersonal communication skills.
- Demonstrated ability to _work_ with mínimal supervision, and collaboratively as a member of a multidisciplinary team.
- Ability to effectively prioritise and organise own _work_ / caseload.
- Demonstrated commitment to professional development, quality improvement and evidence-based practice.
