Aged and Rehabilitation Care provides a range of integrated services, predominantly to support older people reach and maintain health independence and wellness in the community.

The service operates from six Monash Health community sites in the Greater Dandenong, Casey and Cardinia Shire local government areas, as well as from the Kingston and Casey hospital campuses. We provide:
Inpatient care across nine wards at Kingston Centre and Casey Hospital, with a total of 253 beds.

Inpatient care in patients’ homes (Monash at Home) with 40 beds to expedite ward discharges from our hospital campuses to cover the entire Monash Health catchment.

Community specialist clinics that care for priority populations (Movement Disorders, Young Adult Transition Service, Cognitive and Dementia Memory Service, Clinical Gait Analysis Service, Falls and Balance Clinic, Pain Clinic, Continence Clinic, National Disability Insurance Scheme)

Community Rehabilitation services across the full catchment (five centres) and offer time-limited interdisciplinary care to promote independence within our communities. Services are offered at home, at one of our centres and/or by tele-rehab consultations, and can be delivered in a group or individual setting.

Commonwealth-funded aged care programs in the community, including the Aged Care Assessment Service (ACAS), Regional Assessment Service (RAS), Home Care Packages (HCP) and Commonwealth Home Support Program (CHSP) and Transition Care Program (TCP). TCP provides short-term care after a hospital stay with 48 contracted care beds through a private residential aged care facility, and an additional 35 places to support clients in their own homes.

**About Residential In Reach**

**Residential In Reach (RIR) services provide an alternative to presenting to an Emergency Department, where appropriate and safe, for people living in Residential Aged Care Facilities (RACF). The services provide assessment and management (in appropriate circumstances) of acute medical conditions that would otherwise result in a resident unnecessarily going to hospital.**

**RIR services are staffed by hospital-based nurses and doctors. RIR services include**:

- Telephone consultation and liaison with RACF staff, general practitioners (GPs) and other providers.
- Assessment and management of residents within the RACF in collaboration with RACF staff and the resident's GP.

**About the Role**

Consultant Geriatrician required to provide geriatric medical services to Residential InReach and LSL cover within the Aged and Rehabilitation Division to the CDAMS service. You will work with multidisciplinary teams comprising of Consultant Geriatricians, Advanced Trainees in Geriatric and General Medicine, nursing and allied health. You will be responsible for providing patient centred, evidence based care to patients within residential aged care settings and outpatient clinics. Delivery of care utilising both face to face as well as telehealth options as required.
- Provide clinical leadership through active involvement in patient care.
- Be available to medical colleagues for consultation and liaison as appropriate regarding patient care.
- Contribute to the professional and corporate life in Victoria's largest public health service by participation in appropriate committees and educational platforms.
- Contribute to ongoing service improvement and clinical and workforce innovation to ensure Monash Health provides high quality accessible Rehabilitation Medicine services within allocated resources and meets the needs of Monash Health patients.
