Intake Officer

Mansfield District Hospital is seeking a highly motivated person will support our hard working and dedicated team of Primary Care Centre and Visiting Nursing Service by identifying our client’s initial needs through intake assessment, provide them with information on service options, determine eligibility for health services and allocate the referral to the appropriate worker within the Primary Care Centre or Visiting Nursing Service.

Delivering Great Care to our clients is central to this role. The successful applicants will have a verified interest in the service delivery provided by our Primary Care Centre and Visiting Nursing Service, have exceptional administrative and planning skills and be highly motivated to deliver Great Care. A working knowledge of My Aged Care and the National Disability Scheme (NDIS) would be an advantage but not essential and experience in booking or client intake processes an advantage.
