**About us**
Our
**Stafford** Home and Community team is key in providing care to a range of older clients. Our teams provide support and services that enable our clients to maintain their independence - living happily and healthily within their own homes.

**About the role -**
As a Client Liaison Officer, you will provide management of the care for up to 50 clients, who receive varying levels of Home Care packages. Working as a vital member of a multidisciplinary team you will ensure that the client is receiving the required care to remain living within their own homes.

Case management experience is desirable for this position as is being competent in using computers and having strong problem-solving skills.

You will enjoy building rapport with our clients and being part of a dedicated professional team as you undertake assessments, support planning, packaged care coordination and case management activities designed in collaboration with the client to assist the client in achieving their identified goals.

**bout you**
Ideally you will have a background in Community Services, Aged Care or Disabilities.
- Certificate IV in Health, Community Services, Aged Care or Disability Support
- Proven understanding of assessments, packaged care and case management
- Ability to engage clients and work effectively with them to achieve identified goals.
- Support and guidance to clients with budget management and financial sustainability
- Exceptional communication skills
- Ability to advocate strongly for your clients.
- A sound understanding of the community care system is highly desirable.
- A current National Police Clearance
- Current Covid Vaccinations
