**About the Position**

**We are looking to welcome a** Family Support Practitioner **to deliver an outreach model of family case-management programs to families.**

- **Part time (0.6FTE) position with****:

- **Max term role until 30 April 2025.****:

- **Level 4 under the SCHCADS Award. Salary range is $85,127 - $91,588 (pro-rata) + Super.***

- **Engage families and develop case plans which address unmet needs and child protection concerns.****:

- **Work with other professionals and agencies utilising an ecological model.****:

- **Facilitating social learning, community connectedness, good health and wellbeing.**

**About You**

***

***:

- **A tertiary degree in a relevant discipline (allied health, social work, human services).****:

- **At least 2 years’ experience providing case-management and therapeutic services to families, caregivers, young people and children in a child protection framework.****:

- **Cultural awareness and an understanding of how to communicate and work effectively with families from different socio-economic and cultural backgrounds, including Aboriginal and Torres Strait Islander people.***
