**Case Manager - Aged Care clients & NDIS clients**

HomeCare Australia is a leading family owned and operated organization and provides services to assist the elderly and disabled to stay living independently in their own homes for longer with homecare packages with NDIS services. As a result of the strengthened reforms, we seek individuals to join our case management team.

This role involves solving problems for your clients, organising services for them as well as formal case management reviews. You will also be responsible for incoming calls and solving issues immediately.

To be successful in this role you will have a strong customer service focus, be a proven problem solver, show initiative and retain your clients over time. We seek someone with a medical background preferably; customer service experience and someone who has worked in an environment where problem solving is a given.

All our office staff currently work from home so you will need an office and high performing NBN.

**Essential Skills and Development Required**
- Demonstrated critical thinking skills
- A high standard of teamwork
- Attention to detail and follow through
- Diploma in community services or equivalent experience with managing cases
- Demonstrated significant experience and knowledge of the needs of older people and people diagnosed with dementia as well as those suffering a disability
- Experience with cloud based databases and Microsoft office including google drive
- Commitment to providing quality aged care within the community
- Excellent oral and written communication skills
- Ability to follow HCA policies and procedures
- Current drivers license
- Federal Police Check
- Current NDIS worker screening

To be successful in this role you will need a high degree of drive coupled with a passion for helping our most vulnerable citizens.
