**Your Role**

As a Dementia Advisor you'll be supporting the most important people in our organisation - our clients.

Your tasks will include:

- Providing person centered dementia advice to people living with dementia at home and their carers
- Providing a high level of assessment, behavioural analysis, goal planning and enabling strategies verbally and in written reports
- Care co-ordination and referrals to local support services
- Evaluating the effects of support provided through planned reviews
- Contributing to service planning, management and reporting tasks
- Marketing and networking with community groups and individuals
- Delivering dementia education packages to the community and service provider.

**Role Requirements**
- Proof of immunity, or willingness to become vaccinated against, Measles, Mumps, Rubella (MMR), Varicella (Chicken Pox) & Pertussis (Whooping Cough).
- Current Police Screening Certificate
- Current Queensland driver's licence
- Tertiary qualifications and/or equivalent experience in a health related field
- Certificate IV in Training and Assessment or working towards this qualification
- Demonstrated experience in the delivery and management of community-based dementia services and care
- Current First Aid and CPR Certificates
