**What You Will Be Doing**

This position sits within Peninsula Health’s (PH) AOD Program and is based at our Community Health site, The position involves supporting complex AOD client’s throughout their treatment journey using a care coordination approach. It involves providing individualized and flexible support that supplements clients’ AOD treatment, and liaising with other key staff and services that are involved in the clients care. The role also involves facilitating referring on to appropriate internal and external services and programs.

**What You Need**

We anticipate you will have:

- Qualification appropriate to the position such as Certificate IV, or Diploma in Alcohol and other Drugs or Postgraduate course in Addiction
- Demonstrated experience working with at-risk and vulnerable people with dual diagnosis
- Demonstrated experience in working both independently and as a part of a multi
- disciplinary team, with the ability to work effectively, supportively and flexibly
- Demonstrated knowledge in providing care coordination or case management to complex and high risk clients
- Demonstrated commitment to professional development and evidence-based practice.
- Well-developed written, verbal, interpersonal and computer literacy and communication skills
- Demonstrated ability to manage multiple competing tasks and effectively prioritise
- Current Victorian drivers licence.
