About the Role
- Deliver specialised nutrition and dietetics clinical and consultancy services to consumers accessing care in the chronic disease service stream in Community and Oral Health, as part of the multi-disciplinary community-based care model (inc. caseload related to diabetes, pulmonary rehabilitation and cardiac rehabilitation.
- Demonstrate high-level clinical leadership and mentoring support to problem solve urgent and emergent complex clinical issues.
- Foster a culture that delivers safe, patient-centred and compassionate care considering each patient's psychological, social, emotional, spiritual and physical care.
