**About the facility**

Carer Support provides assistance to unpaid family carers of people who have a chronic illness or disability, a mental health condition or who are palliative or frail aged. The team provides support to individual carers with service coordination, provision of practical and emotional supports and assistance to build capacity and improve their wellbeing.

**About the role**

To coordinate services across Barwon Health Carer Support, including Carer Gateway, Support for Carers Program, and Community Home Support Program (CHSP) ‘care relationships’. The coordinator will assist the Community Health Program Manager in the planning, implementation, coordination and evaluation of systems and processes to ensure high quality services which are consumer centred. A large focus of this role is ensuring the consumer/carer journey meets the core principles of access, consumer centred care and support for ongoing well-being.
