The role you could play

We are seeking a proactive Case Manager You will be responsible for the provision of high-quality case management support within the Family Housing & Support Team by providing support to families and children who are homeless or at risk of homelessness.

Duties
- Effective relationships are established with clients that encourage self-determination and participation
- Case management support, including outreach support, is provided to clients in accordance with TSA case management models and standards of practice
- Ongoing assessment is made of the client’s issues, needs and circumstances from initial intake to exit
- Individual case plans are developed in collaboration with clients in a timely manner and include a range of interventions
- Progress of individual case plans are regularly monitored and where needed action is taken to ensure the plan continues to meet client needs or goals

This is a permanent full-time position, 38 hours per week, based in Sunshine, VIC. Salary and conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award Level 5.

About you
- Tertiary qualifications (Bachelor) in Social Work or related community service field
- Experienced in Case-Management
- Experienced with families who are homeless or at risk of homelessness
- Experienced with working with Culturally and linguistically diverse (CALD) communities
- Ability to understand and engage with people from CALD backgrounds, and work with Interpreter services
- Demonstrates knowledge of exit
- planning options and when to implement.
- A Current Victorian Drivers Licence
