**Job Summary**:
We are seeking a dedicated and compassionate Psychosocial Recovery Coach to join our team. In this role, you will play a fundamental part in providing assistance and guidance to individuals with disabilities, supporting their psychosocial recovery journey. You will work closely with our customers, helping them to identify their goals, develop personalised recovery plans, and access appropriate support services.

**Responsibilities**:

- Conduct comprehensive assessments to identify individual strengths, goals, and support needs.
- Collaborate with customers to develop personalised recovery plans and set achievable goals.
- Provide guidance and support to customers in navigating various support services and resources available and relevant to the participant.
- Assist customers in building and maintaining relationships with healthcare providers and support service providers.
- Monitor progress and regularly review recovery plans to ensure their effectiveness.
- Advocate for customers' needs and rights, promoting their self-determination and empowerment.
- Maintain accurate and up-to-date documentation of customer interactions and progress.
- Work collaboratively with a multidisciplinary team to coordinate and deliver holistic support services.

**Requirements**:

- Relevant qualifications in psychology, social work, counselling, or a related field.
- Proven experience in supporting individuals with psychosocial disabilities in their recovery journey.
- Strong understanding of person-centered approaches and recovery-oriented practices.
- Excellent communication and interpersonal skills, with the ability to establish rapport and build trusting relationships.
- Ability to assess needs, develop personalised plans, and implement effective support strategies.
- Knowledge of relevant community resources, support services, and benefits available to individuals with disabilities.
- Empathetic and non-judgmental approach, with a genuine passion for empowering individuals with disabilities.
- Strong organisational skills and the ability to manage a caseload effectively.
- Valid driver's license and willingness to travel as required.
- Valid Clearances: NDIS Workers Screening and Working with Children Check
