About the role
The Midwife Navigator High-Risk Antenatal Care is a Registered Midwife who demonstrates practice at advanced levels in both knowledge and practice. The Midwife Navigator aims to facilitate a safe and efficient care for women who are high risk through the reduction of service fragmentation, education, coordination and empowerment of individuals to enable self-management. The role will work with women who have pre-existing diabetes, complex co-morbidities, previous birth trauma, barriers to attending care, and/or are identified as a high risk obstetric patient within a multidisciplinary team.

About you
- Advanced clinical evidence-based skill and knowledge in, midwifery, high risk antenatal care, coordination and interdisciplinary care management, evidenced by recency of practice and achieved clinical and professional competencies.
- Demonstrated ability to build and maintain strong relationships and to develop strong teams across the health landscape within a framework of open communication.
- An ability and commitment to deliver clinical services collaboratively integrating the broader multidisciplinary health care team across the system in delivering high quality, patient centred care.
- Demonstrated contemporary clinical nursing expertise with clear understanding of the requirement for evidence-based patient centred care.
- Demonstrated ability to work autonomously and provide independent professional judgement and expertise in patient centred problem solving and clinical management.

**Mandatory Requirements**:

- Vaccinated against, and remain vaccinated against, certain vaccine preventable diseases (VPD) as outlined in the Health Employment Directive NO. 01/16
