Tweed Community & Allied Health have a permanent full time position for a Clinical Nurse Specialist Grade 2 (CNS2) to join their Hospital in the Home team, coordinating and delivering high quality nursing care to patients/clients and providing clinical leadership and specialised clinical practice in an integrated model of nursing care called Hospital in the Home (HITH), which is designed to prevent hospital admissions and support shorter length of inpatient stay. This service is delivered across the community and acute interface and incorporates medical clinical governance, patient and staff safety and operates within the parameters of Local Health District (LHD).

**_If you have/are:_**
- Current registration with the Nursing and Midwifery Board of Australia as a Registered Nurse (Division 1) with a post graduate qualification in a field clinically relevant to Hospital in the Home and at least 3 years' clinical experience in the provision of Hospital in the Home (HITH), community ambulatory care services, and/or acute inpatient setting relevant to HITH.
- Clinical leadership skills and the ability to work collaboratively within a multidisciplinary framework to plan, deliver and coordinate patient care
- Commitment to excellence in nursing practice, the provision of education to others and own professional development
