Case Manager

**The Role**:
A Case Manager is required to provide support and advice to Home Care Clients to assist them to continue living independently. You are required to conduct assessments, care planning and navigate directed services to complex care clients within budget guidelines. This is an extremely rewarding role where you will liaise and build relationships with the client, their families, medical and allied health professionals as well as other service providers. We currently have work in the Macedon Ranges and surrounding towns, including Castlemaine and Sunbury. **About You**:
- You will have a genuine passion for working in Aged Care.

- You are empathetic, resilient, have outstanding personal qualities, and have a preference for working within a collaborative team.
- You can interact, find common interests and develop a connection with the older community.
- You understand that you are working for an organisation that is committed to supporting you, and in turn, you will support our clients.
- You have Tertiary qualifications in Nursing, Allied Health, Social Work, Aged Care, Care Management, and/or related experience.
- You have previous experience in Aged Care services and an understanding of the Aged Care industry, including Home Care Packages and the Aged Care Quality Standards.
- You are an effective communicator with exceptional customer service and problem-solving skills (innovative and creative)
- You are efficient and organised with proven time management skills
- You have good IT skills, including the Microsoft Office suite
- You can work independently and as part of a team
- You have experience in managing client budgets - desirable but not essential.
- You hold a Victorian Driver's License, have reliable car and car insurance.
- You hold a current Police Check
- You have evidence of having the COVID-19 vaccination
- You have values that align with ours, 'respectful, ethical, and caring".
