**The opportunity**

Working as part of our multidisciplinary healthcare team, you’ll support the delivery of health care for our clients across Australia. Your approach will be driven by human-centered, holistic, evidence-based care.

**Your New Role**

Our Home Health program delivers in-home support services to our customers to help them live independently, supported, and empowered, within their own homes and communities.

The Community Connect Specialist role would see you prioritizing client services against staff capacity, schedule community nurses and support the admin team with the daily function of the office.

**What does your new role look like?**
- KPI Scheduling client services 10 business days in advance
- Priorities client intake against staff capacity
- Allocation of client services PPI 12-17 per day per day
- Building and maintaining strong and effective relationships internal and external providers
- Work independently and collaborative with the Brisbane team
- Adherence to health and safety guidelines and policies
- Ability to multi-task and manage your time effectively in a fast-paced environment with a high attention to detail

- Previous scheduling experience
