**A bit about you**:
You are an enabler who successfully engages, supports and coaches disengaged young people by providing critical interventions.

You hold a Certificate IV in Mental Health, Community Health or a related discipline and have experience working with young people (12 - 24) in case management and brief intervention support with a focus on mental health and wellbeing outcomes.

You’re passionate about working with at-risk and vulnerable young people and you know your stuff when it comes to guiding and collaborating in a flexible, youth-orientated environment.

You know not every day will be the same, you can switch from providing support to a service user, to writing impact reports for your manager in the blink of an eye. Changing lives is your passion but you still embrace rolling up your sleeves and mucking in to get the job done.

You’re full of creative ways to meet our service users where they are at and believe that engagement shouldn’t be a one size fits all model. You always see the person, not just their immediate situation because you understand that everyone has different needs, and that is okay.

**A bit about the role**:
YHOP is an assertive outreach service that engages/connects at risk young people to relevant referral pathways to increase engagement, wellbeing and social participation in their community, overcoming barriers.

This is an exciting position in the front line of youth assertive intervention where outstanding motivation and passion will make the most difference. As the first point of contact for at-risk youth across northwest Melbourne, this position is critical in providing positive health, social and economic outcomes for young people aged 6-24 years. This position plays a pivotal role in linking clients to other supports and services internal to Youth Projects and initiatives across the Northwest of Melbourne.

**Role Type**: Part Time (22.5 hours per week)

**Key Requirements**:

- Certificated IV or Diploma qualifications in relevant mental health, community services and/or health related disciplines
- Direct service working with clients experiencing multiple and complex needs
- Drivers Licence

We are an independent, registered charity providing front line support to young people and people experiencing homelessness to open up pathways out of poverty and homelessness - breaking the cycle of disadvantage. Our focus is on respect, client-centred care, and harm prevention so people can lead longer lives, have improved health, meaningful employment, and a greater sense of belonging, safety, and security through our holistic model of care.

In 2021 alone, this is (some) of what we got up to:

- Over 8,000 people accessed our services
- Welcomed 16,431 visits to The Living Room
- 80% of the young people we supported landed sustainable employment or education outcomes
- More than 150,000 coffees were sold through our social enterprise cafes with $73K profit re-directed into homelessness and youth services
- Experienced 130,000+ contacts through our harm minimisation programs
