**SACARE** is currently looking for a dynamic Senior Clinical Case Manager - RN who is passionate about a client-centred approach to care.This is a newly created opportunity within the organisation and will be responsible for leading a team of two (2) experienced Case Managers, as well as providing hands-on nursing services. This role will support **SACARE’s** in-home services.

**About you**:

- AHPRA Registered Nurse (unencumbered license)
- Have a NDIS workers check, WWCC or willingness to obtain
- Have a minimum of 5 years of nursing experience, with an in-depth understanding of community nursing
- Hands-on clinical leader, with demonstrated evidence of providing great client service
- Can assess client needs and create care plans in collaboration with the client and stakeholders which are clear, concise and meets their needs.
- Have a demonstrated knowledge of advanced nursing supports, including wound care, gastrostomy care, chronic conditions management
- Have a sound understanding of the NDIS & other funding streams i.e., DSOA, LSA etc-** essential**:

- Experience in the disability or health sector and an understanding of the issues experienced by people with disabilities and their families-**essential**:

- Demonstrates advanced knowledge of risk assessment and incident reporting
- Uses a high level of professional knowledge and expert skill to provide assessment, interpret findings, and plan and implement intervention in collaboration with clients, carers and other health professionals.
- Can respond promptly to client feedback and reports regarding client issues and managing outcomes.
