**About the role**
We are seeking a dedicated and experienced Support Coordinator to join our team in Canterbury. This full-time role requires you to be in the office for the first 2-4 weeks for training. Following this period, a hybrid or remote work model will be available.

As a Support Coordinator you will work closely with clients to effectively utilise their NDIS-funded supports, connect them with suitable service providers and allied health professionals, and monitor their progress towards achieving their plan goals.

**Responsibilities include**:

- Provide support to clients and their families in accessing and coordinating services.
- Build collaborative relationships with internal and external stakeholders.
- Facilitate the implementation of all supports outlined in NDIS plans.
- Enhance clients skills to coordinate support and engage actively in the community
- Empower clients with a deeper understanding of their plans and how to implement them
- Be available and responsive to client needs, especially during crises.
- Conduct regular progress reviews of client plans, ensuring timely and accurate reporting to the NDIS.

**Skills, Qualifications and Experience**:

- Excellent interpersonal and communication skills
- Strong written communication skills
- Demonstratable problem-solving skills
- Ability to collaborate effectively
- Understanding or knowledge of the NDIS
- Experience in support coordination or similar
- Familiarity with psychosocial disabilities (advantage)
- Relevant qualifications in disability services, social work, or related fields (desirable)

**Requirements**:
The successful applicant will have or be willing to obtain;
- Driver’s Licence
- Working with Children Check (WWCC)
- NDIS Worker Screening Clearance
- NDIS Worker Orientation Module certificate
- CPR + First Aid certificate
