An exciting opportunity has become available for a passionate **Assistant Care Coordinator** who is looking to join a supportive team and make a positive impact on the lives of their clients. Based in their new Gregory Hills office this is a Monday to Friday role working amongst a supportive team.

**About the Role and your Responsibilities**:
Your role as an Assistant Care Coordinator is to support the Care Coordinator in supervising a team of support workers and coordinating the care of clients. You will provide support to the Care Coordinator and the team to assist in the coordination of support. This role is a pathway to becoming a Care Coordinator. Some of your responsibilities will include:

- Contribute to the development of care plans
- Risk management plans,
- Updating client files
- Administration and assist with client onboarding
- Working closely with the team to deliver a high quality services
- Supporting staff in their role to deliver and maintain high standards of care

**Skills and Qualifications**:

- Certificate III in Disability or other relevant qualification
- Ability to create assessments and reports effectively
- Demonstrated understanding of the needs of people living in the community
- Proven customer service, administrative and organisational skills
- Computer literate and have the ability to learn new programs quickly
- Excellent interpersonal and written communication skills
- Conflict resolution skills and the ability to handle challenging situations in a professional and sensitive manner
- Ability to work in a very busy environment and to work both independently and as a part of a team
- NDIS worker’s check
- National Police Check - Optional
- Current Working with children’s check is desirable
- Valid First Aid & CPR certificate
- Covid Vaccination Certificate (showing vaccination dates)
