**The Opportunity**

As a Transition Case Worker in our Youth Pathways Program, you’ll be at the heart of a high-impact team. You’ll use holistic case management to provide specialist support for young people (10 - 24 years) with complex needs who are leaving NSW Correctional Centres and Youth Justice Centres.

Using a throughcare model focusing on providing a continuum of support aimed to address the complex and diverse needs of children and young people, your role includes:

- **Client support**: Providing holistic, trauma-informed case management with an AOD focus will make up most of your day. You’ll actively build positive relationships with Government, community and other services to source relevant support and options for young people (10 - 24 years.). You'll assess new referrals and maintain client files. You’ll approach every task with empathy, keeping our clients’ goals front and centre.
- **Creating community connections for clients**: Using an outreach model pre
- and post-release, you’ll play a vital role in providing clients with practical and psychosocial support. The goal? To meet the young person's identified needs, ensure access to long-term supports, reduce incarceration and reduce recidivism.
- **Data, administrative and assessment duties**:While the majority of this job involves client-facing duties, data collection will be an important part of your role. You’ll conduct general administrative duties, monitor workplace health and safety, and update and maintain accurate client files.
- **Cultural safety**: You’ll work in culturally appropriate ways in recognition of the unique cultural needs and over-representation of First Nations young people in the criminal justice system. You will support clients and their families to have their cultural needs met.

Your usual work hours will be Monday to Friday, with occasional out-of-hours work, as required.

**To join us, you’ll need**:

- To identify as Aboriginal or Torres Strait Islander
- A relevant tertiary qualification (minimum Cert IV) in Community Services, Social Work, or related field
- Knowledge and experience of case management with children and young people with complex and compounding needs such as criminal justice system involvement, homelessness, alcohol and other drug use, mental health and experiences of trauma
- Experience and skills in the provision of trauma-informed care
- A current NSW Driver Licence
- A current NSW Working with Children Check and Criminal Record Check (please note, a criminal record is not a barrier to being considered for a role with CRC)
- Demonstrated cultural competency and knowledge of issues relating to Aboriginal and Torres Strait Islander and CALD communities
- Knowledge and skills relating to child development, current child protection issues and legislation.
