**Position Overview**:
The Support Coordinator plays a crucial role in assisting individuals with disabilities to navigate the complex landscape of support services and resources. Working for Assist Ability Australia, the Support Coordinator is responsible for facilitating access to a range of supports and services, empowering individuals to achieve their goals and enhance their quality of life.

**Key Responsibilities**:

- **Client Support**:

- Provide person-centered support to individuals with disabilities, understanding their unique needs, preferences, and goals.
- Assist clients in identifying their support needs and developing individualised support plans to achieve their objectives.
- Advocate for clients to ensure they have access to appropriate services and resources, including disability support services, healthcare, housing, and community programs.

- **Service Coordination**:

- Liaise with service providers, government agencies, and community organisations to coordinate services and supports for clients.
- Monitor and review the implementation of support plans, making adjustments as necessary to ensure client needs are met effectively.
- Facilitate referrals to specialist services and support programs based on client needs and preferences.

- **Documentation and Reporting**:

- Maintain accurate and up-to-date client records, including support plans, progress notes, and service agreements.
- Prepare reports and documentation as required by funding bodies, regulatory agencies, and internal stakeholders.
- Ensure compliance with relevant policies, procedures, and legislative requirements in all documentation and reporting activities.

- **Client Empowerment**:

- Provide information and guidance to clients to build their capacity to self-manage their supports and make informed decisions.
- Support clients to develop their advocacy skills, enabling them to assert their rights and preferences in accessing services and resources.
- Foster a collaborative and empowering relationship with clients, promoting their independence and self-determination.

**Qualifications and Experience**:

- Bachelor's degree or equivalent qualification in a relevant field such as social work, disability studies, or human services.
- Previous experience working in disability support coordination or a related role is highly desirable.
- Knowledge of the National Disability Insurance Scheme (NDIS) and experience in navigating disability support systems and services.
- Strong communication, interpersonal, and advocacy skills, with the ability to build rapport and establish trusting relationships with clients and stakeholders.
- Excellent organisational and time management abilities, with the capacity to prioritise tasks and manage competing demands effectively
