**About Us**:
At Mt Gravatt Community Centre / Queensland Ageing and Disability Services, we are driven by our mission to provide dynamic, client-focused care that makes a real difference in the lives of older Queenslanders. Our multidisciplinary team is committed to delivering best-practice, home-based programs and services that support clients in their own homes, allowing them to live independently and with dignity.

Our programs are designed to support early hospital discharges and prevent unnecessary admissions by providing comprehensive in-home care.
**Key responsibilities include**:
- **Clinical Leadership**: Deliver high-level clinical care, including wound care, fall prevention, and risk assessments.
- **Case Management**: Oversee initial and ongoing client case management, ensuring care plans are adjusted to meet clients’ evolving needs.
- **Team Collaboration**: Work closely with medical professionals and allied health services, including Nurse Navigator programs, as well as other QAADS team members.
- **Client-Centred Care**: Monitor, implement, and modify care plans in collaboration with clients and families, empowering them to achieve personal and clinical goals.
- **Policy Development**: Support the development and improvement of policies and procedures for care workers to ensure best practices and compliance with industry standards.
**What We’re Looking For**:

- A client-focused approach with an ability to manage priorities in a fast-paced environment.
**Desirable**:
- Two years of experience with aged care and home services.
- Knowledge of relevant industry regulations and quality standards.
**What We Offer**:
- A supportive and dynamic team environment where you can grow both professionally and personally.
- Opportunities to develop and refine your leadership skills in an innovative, client-centred care setting.
- Salary packaging options to improve your take home pay - up to $18,500 can be packaged every year
- Leave loading of 17.5%
- Access to Employee Assistance Program (EAP)
- A work-life balance with flexible working arrangements where possible.
- A competitive salary range, in accordance with the SCHADS Award, with the final remuneration based on your experience, skills, and qualifications.
