Care Coordinator

**ABOUT THE ROLE**:
**Summary of the role**:

- Full time position
- Hughesdale office-based role
- Excellent communication skills
- Facilitate care and services for Mercy Health Home Care Package and other Aged Care related funding streams
- Strong team player
- Be part of a multi-disciplinary team which requires you to provide highly responsive and flexible services to consumers and carers

**ABOUT YOU**:

- Prior experience in community aged care
- Demonstrated time management and organisational skills
- High standard of verbal and written communication skills
- Genuine care, understanding and commitment supporting quality aged care
