**Description**:

Full time contract until 30/06/2025, based in Geelong/Barwon region

About You

**Additional attributes we are looking for include**:
Previous experience in assertive outreach, youth work, case management, mental health or AOD (professional, volunteer, lived experience); or qualifications in psychology, mental health, social work or other relevant fields
The ability to effectively and quickly build rapport with people
Flexibility, patience, confidence, initiative and emotional resilience
Effective listening skills & and a strong but adaptable communication style
The ability to contribute positively within a team and work autonomously to manage your case load

It is a requirement of this position to be fully vaccinated against COVID-19 and meet all compliance requirements in relation to NDIS Work Screening, Working With Children’s Check, work rights and possess a current & valid driver’s license.

About the role
ermha365 provides the Early Intervention Psychosocial Support Response (EIPSR) program in partnership with Barwon Health. EIPSR provides specialist psychosocial support to help eligible participants to build their capacity to better manage their mental health, develop practical life skills for independent living and social connectedness, and if eligible, transition to the NDIS.
The Support Facilitator’s goal is to enable rather than to do; to promote independence, development and confidence in the participant. The Support Facilitator role further entails:
Provide psychosocial support to young people and adults living with severe mental illness on a short to medium term basis (up to 12 months)
Work with participants, their clinician and other stakeholders (e.g. family) to access and engage with a range of supports
Develop, monitor and track recovery goals and implement steps to work towards independence
Complete screening and assessments, and participate in case plans and exit reviews in collaboration with care teams and clinical staff
Advocate for others and build support networks

This role works on a hybrid system, including working from home, in our North Geelong office and assertive outreach support to participants across the Greater Geelong region and Bellarine Peninsular.
Program hours are typically 8am to 4:30pm (some flexibility). This is a contract position ending 30/06/2025.
