**About the role**

We are looking for an experienced **Australian Male SW**with a who is hard-working, physically fit and dedicated to join our team. You will need to be friendly, positive and demonstrate client-focused behaviour, showing empathy and treating others with dignity and respect. You’ll also need to have a natural ability to build relationships that make a difference, be honest, helpful and dependable, making you a great asset to our team.

As a Disability Support Worker, you will play a crucial role in providing personalised support to individuals with disabilities, with a focus on those with mental health needs.

Your work is based in a Participant's house and in the Community.

**Positions available**:

- In-home support / Assistance with Daily Living, Community Access and Transport - (Monday-Friday & Weekends)

**Position Overview**:
As a Support Worker at Kuremara Care, you will play a crucial role in providing support to the client. We are seeking a compassionate and dedicated **support worker** to join our team.

- Ability to work independently and as part of a team.

**Responsibilities**:

- Provide one-on-one support and assistance to individuals with diverse needs.
- Working within a person-centred, enabling environment that respects the rights and needs of each client
- Assist with daily living activities, including personal care, meal preparation, household tasks and domestic duties
- Assisting clients with daily errands, companionship and support
- Providing support with academic, social or recreational activities
- Preparing food, feeding clients and assisting with medications
- Observe, care and report any changes in the health and well-being of the client
- Engage the individual in social and recreational activities, fostering independence and community integration.
- Maintain accurate records and documentation as required.
- Collaborate with a multidisciplinary team to ensure the individual's needs are met effectively.

- Community Participation**: Facilitate and engage in community activities to promote social integration and participation.
**- Personal Care**:Assist with daily living activities, ensuring the comfort and well-being of the clients.
**- Transportation**: Safely transport clients, creating a supportive and secure environment.
**- Appointment Assistance**: Accompany clients to appointments and provide necessary support.

**Requirements**:

- Experience**:Proven experience as a Mental Health/ Disability Support Worker, with a strong background in community participation and personal care.
**- Qualifications**: Relevant qualifications in disability support or a related field.
**- Communication Skills**:Excellent communication and interpersonal skills to build rapport with clients and work effectively within a team.
**- Driver's License**:A valid Australian driver's license is essential for transporting clients.
**- Availability**: Flexible availability, including weekends.

**Essential Criteria**:

- Excellent verbal and written communication skills
- Willingness to learn, develop and take on new challenges

- Understanding of safe work practices.
- Previous employment experience in the health/Disability care sector.
- Ability to work independently and accurately report back to supervisors.
- Flexibility to work various shifts, including evenings and weekends.
- Experience in providing quality services that assist people to maintain their independence, dignity, cultural values and religious beliefs.

**Qualification**:

- Support worker: At least 2 year of experience
- NDIS Worker's Orientation
- NDIS Check - Yellow Card
- Working with Children Check (Blue Card)
- Police Check
- Current First Aid & CPR
- Passport or Visa
- Australian Driver Licence
