HARP Care Coordinator
Permanent Part Time - 48 hours per fortnight

**Classification**: Community Health Nurse (ZJ1)

**Award**: Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2020 - 2024
This is an exciting opportunity for a suitably qualified community health nurse to join the Community Services team as HARP Care Co-ordinator. In this role, you will be responsible for providing care coordination for complex clients, particularly medical complexity.

KEY SELECTION CRITERIA
Essential

**Qualifications**:

- Allied Health or Nursing qualifications and relevant registrations
- Current Victorian Driver's License

Technical/Professional Knowledge and skills
- Understanding and commitment to person-centred care and chronic disease self-management.
- Ability to use clinical knowledge and skills to identify and address opportunities for service improvement and innovation.
- Ability to work effectively across the continuum of care for people with complex healthcare needs

Personal Attributes
- Ability to communicate in a timely and effective manner.
- Ability to work independently and as part of a team.
- Ability to build partnerships with a range of different professionals.
