Role is to undertake outpatient clinic coverage only.

**Roster - **Monday to Friday 0800 - 1630. No weekend shifts. **Oncall** (1630 to 0800) rostered for 25/9, 26/9, 27/9, 28/9, 9/10, 10/10, 11/10, 12/10
- **START**:Monday 30 October 2023 (0800)
- **FINISH**:Friday 11 November 2023 (1630)

Role is to undertake outpatient clinic coverage only.

**Roster - **Monday to Friday 0800 - 1630. No weekend shifts. **Oncall** (1630 to 0800) rostered for 6/11, 7/11, 8/11, 9/11
- **START**:Monday 4 December 2023 (0800)
- **FINISH**:Friday 15 December 2023 (1630)

Role is to undertake outpatient clinic coverage only.

**Roster - **Monday to Friday 0800 - 1630. No weekend shifts. **Oncall** (1630 to 0800) rostered for 11/12, 12/12, 13/12, 14/12
- **START**:Wednesday 27 December 2023 (0800)
- **FINISH**:Sunday 7 January 2024 (1630)

Role is to undertake outpatient clinic coverage and weekend ward round.

**Roster - **Monday to Friday 0800 - 1630. Weekend shifts (0830-1030 ward rounds) Sat 30/12, Sun 31/12, Mon 1/1

Oncall (0800-0800) for 30/12, 31/12, 1/1 and **Oncall** (1630 to 0800) rostered for 2/1, 3/1, 4/1
- CV - Must be signed and dated. Job history with current role first and any gaps identified. Also include qualifications and AHPRA registration details.
- References - two written references that are to be within 12 months old and at least one from a current position
- ID - please include current DL or passport

**Accommodation**

Accommodation will be provided by Toowoomba Hospital if available.

**Travel**

Return economy travel to and from Toowoomba is provided. If locum is driving then mileage is to be charged at a pre-prescribed rate.

**Medicare**

It is mandatory for the locum to have a provider number prior to the locum commencing..

- AHPRA Registration - **_must be current_**:

- Australian National Police Check - **_must be current, valid for 3 years_**:

- Immunisation - **Measles, Mumps, Rubella, Varicella, Hep B, Pertussis (must be from within the last 10 years) ADT will not be accepted nor will pertussis serology.**:

- **Vaccine Preventable Diseases**:

- Vaccine Preventable Diseases Form including evidence of vaccines/immunity
- Tuberculosis risk assessment questionnaire
- Proof of 3 COVID Vaccinations
- Proof of 1 Flu Vaccination
- Photo Identification - **_current passport or driver’s license with clear legible photo and writing - if it has expired, this is not acceptable._**
