KompleteCare are looking for Case Managers that will support our client base within the Aged Care and NDIS sector.

Applicants MUST hold either a Diploma in Community services, Diploma in Nursing, Bachelor of Nursing or a Bachelor within the industry.

The successful person will work within a supportive team who will offer training and support. It is essentially that the correct applicant has very sound knowledge of Microsoft Office and is efficient on computers.

We are looking for Case Managers at various offices.
- Onboarding clients into the system with appropriate supports upon reviewing medical documentation and plans.
- Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort.
- Monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support.
- Maintains clients' records by reviewing case notes; logging events and progress.
- Communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences.
- Advocates for client needs, choices and rights.
- Respect client’s choice of advocate and includes them as needed in providing and identifying service needs.
- Prepares clients' discharge by reviewing and amplifying discharge plans; coordinating discharge and post-discharge requirements; orienting and training family members; providing resources.
- Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures.
- Improves financial status by analysing results; monitoring variances; identifying trends; recommending actions to management.
- Manages client budgets.
- Comply with and contribute towards the success of the KC Quality Management System
- Meets budget by monitoring expenses; implementing cost-saving actions.
- Updates job knowledge by participating in educational opportunities; reading professional publications; maintaining personal networks; participating in professional organizations.

**Salary**: $75,000.00 - $80,000.00 per year

Schedule:

- 8 hour shift
- Monday to Friday
- On call

Work Location: In person
