The Chronic Disease (CD) Health Coach will work alongside a team of Registered Nurses in a supporting role to assist in managing clients with complex health conditions and chronic disease. The Health Coach will be responsible for delivering person centred motivational coaching, providing a range of current evidence based, best practice health information and support to clients with complex chronic conditions.

The primary focus is to motivate, monitor and inform clients regarding their CD management and wellbeing choices in order to become more active and engaged in the management of their own health.

You will provide **home visits and face to face consultations **to support and assist with tailored health care plans and coordination of support services to ensure clients experience an improved quality of life, lowered rate of avoidable hospital admission and improved access to Primary Health Care.

Success will look like;
- Assisting clients in actioning comprehensive health plans and support them to actively engage in the management of their condition.
- Working collaboratively to assess, plan and implement comprehensive health care plans and individual health and wellness goals.
- Assessing health status and provide person centred coaching and support to help clients self-manage their condition/risk factors and achieve their health goals.
- Guiding clients to build on their knowledge, skills and confidence to support self-management of their health goals and improve health literacy.

**Qualifications**:
We are looking for looking for 2 people who bring allied health skills in either Nursing (Enrolled Nurse) or Allied Health Assistant or equivalent, to combine that passion with helping to create great outcomes with EACH clients living with Chronic Disease and their ongoing management.

You will confident in collaborating with Registered Nurses and other health professionals to together ensure your clients are receiving the best care to ensure the best long-term outcomes possible.

**Mandatory Requirements**
- A current Working With Children’s Check is essential
- Have full Australian working rights and a National Police Clearance
- Australian Drivers Licence
- National Disability Insurance Scheme (NDIS) Check
- Diploma of Nursing or Certificate IV in Allied health
