Working for an approved provider of Home Care Package (HCP), operating across South West and Western Sydney. They are currently expanding and are looking for an experienced **Home Care Manager - Clinical Lead** to join their team based in South West Sydney.

**Job Role and Summary**:
As the Home Care Manager - Clinical Lead, you will report to the Managing Director and Business Development and Quality Manager, and oversee the launch and coordination of the HCP services. You will develop and implement systems, practices, policies, and processes that align with the Aged Care Act and Quality Standards. Additionally, you will ensure that the team delivers quality, safe, and individualised care to clients, carers, and their families.

You will have the energy, experience and leadership capabilities to coach and mentor the team through a projected period of growth, ensuring that the team upholds positive organisational values and culture, services meet legislative and quality standards, and that business objectives are met.

You will be responsible for providing clinical care and guidance to internal and external stakeholders relating to the care needs of the client.

**Responsibilities**:
Duties include and are not limited to:

- Lead and grow a financially viable Home Care service with a focus on quality care and clinical excellence
- Develop and implement assessment tools and tailored care plans
- Ensure the safe and effective delivery of services in line with Aged Care legislation and best-practice
- Provide leadership, coaching and guidance for the Home Care and nursing teams
- Monitor and manage client experience and satisfaction
- Chair the monthly Senior Management Team meeting
- Track and report key program and financial data to the Managing Director
- Maintain the administration practices and procedures in line with the organisations standards

**Skills and Qualifications**:

- Current registration as a Registered Nurse with the Australian Health Practitioner Regulation Agency (AHPRA)
- 2 to 3 years’ experience delivering Home Care or Aged Care services
- 2 years proven leadership experience within an appropriate setting
- Detailed knowledge of the Aged Care Quality Standards and sound understanding of clinical governance, Home Care packages and related legislation
- Experience in the oversight and enhancement of quality, safety and risk management systems
- Experience in budget management, financial reporting and meeting KPIs
- Ability to liaise and consult with a wide range of clients, health professionals and team members
- Excellent documentation and computer skills
- Ability to work autonomously with mínimal supervision
- Current NSW Driver’s license and comprehensive insurance
- Rights to work in Australia
